The Patient Health Questionnaire (PHQ-9) is a brief, 9-item self-screening tool that may help identify symptoms related to depression. The PHQ-9 was developed for use in primary care settings.
Due to the nature of this test, if you do not complete it or have contraindications you will not be able to re-enter and re-test. An error message will appear.

Currently, insurance only covers treatment for those 18 and over. Therefore, anyone under the age of 18 must
pay cash for treatment. We offer a competitive sliding scale for our fees and partner with a third-party medical
lender to help with the financing of treatment.
I do not have insurance
Some insurance companies require patients to be under a doctor's care.
Insurance companies may also require patients to be have tried anti-depressant treatments before they will authorize TMS Therapy.
